Important Information About Reimbursement for Fingerprinting

If you, or your program, have never had payment from the State before and are not in the statewide FMIS system, you will need to submit a W-9 form with the reimbursement packet or your payment will be delayed until the form is on file. If you receive Child Care Subsidy payments, Credential Bonuses, Grant payments, or Tax Returns through direct deposit from the state to your bank account, you are in the FMIS system.  […]
